Life Plan
Everyone needs to establish a strong plan for present and
future services and care delivery. Assessments include a comprehensive review of
all areas of living, status of the care team and services available through
existing and potential benefits. The person in need of care and services
determines the final plan.
Caregivers must seek educational opportunities to become a
better caregiver. Unlike internet resources, a resource specialist is leading
the seminar and allows for interaction. Seminars are structured to fit different
formats from community settings to workplaces.
CRG Resource Specialists are available to consult with caregivers for any situation or question. Inquiries will receive a response within 24 hours. Contacts may include questions on benefits, provider services and relationships.
Family Meetings
Families need to establish which roles they will play in the care team. Having the family members together in one meeting facilitates better communication and for a more solid plan.
